You understand that your information will be kept by Wellness Pharmacy in Canada. You consent to Wellness Pharmacy collecting your personal information from your health care providers to fulfil the prescription, including from the prescriber, the transferring pharmacy, and from your electronic health records. Wellness Pharmacy will disclose your personal information to the pharmacist who fulfils your prescription.
